What is Health Surveillance?
Health Surveillance is about systematically observing early signs of work-related ill health in employees exposed to certain health risks.
It means putting in place certain procedures to achieve this.
Purpose of this Questionnaire:
This questionnaire is part of a Health Surveillance Program that has been put in place by (YOUR COMPANY NAME GOES HERE) for your work health benefit – to make sure you are not developing any health issues as a result of the work that you do.
The information provided on this form will be reviewed by a fully qualified member of ARC nurse staff, who will determine whether or not you will be required to attend a formal work-related medical examination.
