What is Health Surveillance?

Health Surveillance is about systematically observing early signs of work-related ill health in employees exposed to certain health risks.
It means putting in place certain procedures to achieve this.

Purpose of this Questionnaire:
This questionnaire is part of a Health Surveillance Program that has been put in place by your employer for your work health benefit – to make sure you are not developing any health issues as a result of the work that you do.
The information provided on this form will be reviewed by a fully qualified member of ARC Medical staff and a summarised report will be sent to your employer.
This questionnaire is not designed, nor does it act as any part, of a certificate of fitness to work.
